How did this happen?

I noticed an interesting AP "story" in the New York Times. According to the story, the FDA is looking into raising standards for birth control pills because:

The original pills approved in the 1960s allowed on average fewer than one pregnancy for every 100 women taking the pill for at least a year, the drug agency said. But in the last decade, the government has approved pills allowing more than two pregnancies for every 100 women.

Just one problem: the story is not true. I don’t know if the media or the FDA is to blame for the inaccuracy. I just thought it was interesting.
